(A)REVIEW TRANSPORT CANADA REQUIREMENTS

B) ELECTROCARDIOGRAM(ECG):

• If required, PRINT the ECG REQUISITION FORM and take it to DYNACARE LAB(WWW.DYNACARE.CA) at least 1 to 6 weeks BEFORE your appointment;
• If RENEWING an existing medical, ECG MUST be completed and reviewed BEFORE your appointment

(C)AUDIOGRAM:

• If required, available in-office for $95+HST.

• Payment required at the time of booking(credit card 💳);
• No refund for changes or cancellations within 72 hours of appointment;
• Receipt emailed AFTER your appointment;
• Download and use JANE.APP to reschedule your appointment if required;
• Complete the required intake form as soon as possible after booking your appointment.
