Telemedicine Renewal (cat 1, 2, 3)
FEE: $275 CDN
Effective 4 March 2025, only applicants whose previous examination was an in-person appointment will be eligible for telemedicine renewal (i.e. two consecutive telemedicine appointments are no longer allowed)
SCHEDULE YOUR APPOINTMENT
- Payment required at the time of booking(credit card 💳);
- No refund for changes or cancellations within 72 hours of appointment;
- Receipt and FIT FORM emailed AFTER your appointment;
- Download and use JANE.APP to reschedule your appointment if required;
- Complete the required intake form as soon as possible after booking your appointment;
- Must be in Ontario at the time of the telemedicine appointment;
- ECG: NOT required for telemedicine.

Please send via fax or email a SINGLE PDF document of any relevant or required documents before your appointment:
Examples include:
- recent surgeries, injuries, etc
- depression,
- ADHD/ADD,
- diabetes (see resource page for required documents),
- sleep apnea: submit CPAP compliance report,
- laser eye surgery/refractive eye surgery,
- medication list
- Ozempic & Wegovy information
Delayed submission of documents uploaded to Transport Canada after your appointment may incur document management fee of $125+HST.
