Self-scheduling Appointment Steps

F.A.A.

(A) ELECTROCARDIOGRAM(ECG):

• If required, PRINT the ECG REQUISITION FORM and take it to DYNACARE LAB(WWW.DYNACARE.CA) at least 1 to 6 weeks BEFORE your appointment;
• If RENEWING an existing medical, ECG must be completed and reviewed BEFORE your appointment

• Payment required at the time of booking(credit card 💳);
• No refund for changes or cancellations within 72 hours of appointment;
• Receipt emailed AFTER your appointment;
• Download and use JANE.APP to reschedule your appointment if required;
• Complete the required intake form as soon as possible after booking your appointment.

(A)FAA MEDEXPRESS:
Complete your FAA MedXpress information and email your confirmation code to office@pilotMD.ca as soon as possible.

• Submit existing Special Issuance (SI) or SODA documents if required via fax or email as a single PDF file.

(B)REPORTS:
Please fax or email a SINGLE PDF document of any relevant or required documents before your appointment:

Examples include:

• recent surgeries, injuries, etc
• depression,
• ADHD/ADD,
• diabetes (see resource page for required documents),
• sleep apnea: submit CPAP compliance report,
• laser eye surgery/refractive eye surgery,
• medication list
